I think you may have hit the nail on the head here Eion. I think it is a

A long time ago i had a SD card lock up like this as 'read only'.
I think is was due to way I disconnected it from computer.
I have had same with a small USB sticks.
Now i close machine down before disconnecting.
No hot disconnects practised.
It may be different in your case
