fyi, I believe there are two opt-outs
1. Is from the GP
2. Is from the NHS

There is a word file for Opt-out 1

Opt out 2 you can do electronically on the NHS app (not the covid app)
Do you need to complete opt out 1 if you have opt out 2. My surgery
thinks no. I will be completing opt out 1 just in case..
