When you need or don't need a CAL appears to be something of a black art. I heard this week that a network printer getting an IP address from a Windows DHCP server requires a CAL but not if the printer gets an IP address some other way. Even if the Windows server is acting as a print server... The "rule" appears to be that CAL is needed for accessing a "server" on a Windows server. Which can be either "per device" or "per user". (Terminal server and MSSQL have their own special CALs, which are apparently additional to regular CALs). It also appears to be the case that the CAL requirement simply applies to connections. Regardless of if third party software is running on the server. Thus a 3CX system would require regular CALs. Except for softphones running on a device which already had a valid CAL. (If the SIP username wasn't the same as the login username with a "per user" CAL then a softphone might still need it's own CAL.)
