One of the main reasons NHS software doesn't work is because all the people involved in "designing" the software have little or no idea of what is actually required. If they took the simple expedient of talking to the people that used it on daily basis, they'd find out. Of course, that's *far* too sensible. The same applies to software used by the CPS and ONS.
