Not sure if it makes any difference that the / partiton is on an extended partition. IIRC, in the past if Linux was installed on a partition other than the primary partition, a /boot partition was required within the first 1024 cylinders on the drive.Always had the same partition config, in fact it's identical to the desktop partitioning that works fine.
I wonder... if I ghost Windows, blitz the entire HD and re-fdisk the whole lot, if that would help? The Linux partitions were created after Windows was installed.... I wonder if *that* is the problem.
