I believe recent versions of Outlook, allow basic whitelisting (no automated challenge/response yet AIUI), although alas Outlook uses the MS HTML component and is thus riddled with security holes that are driving people away from IE in droves at the moment. Although I think even this may have changed.
The main drawback is that automated email from websites and such often gets caught.
A lot of people rant against challenge/response - most of them have very little grasp of reality in my experience. -----BEGIN PGP SIGNATURE----- Comment: Using GnuPG with Debian - http://enigmail.mozdev.org
