HOW MUCH? Even if they continued to used Microsoft Outlook, or Novell Groupwise, or even another current commericial solution out there, it would cost less. Give them a decent IMAP/LDAP/POP3 solution with something like a Horde IMP server farm would (Guessing) way less than that. The only tricky extras is keep things such as patient records confidential.
